St. Vincent Healthcare is part of SCL Health, a faith-based, nonprofit healthcare organization that focuses on person-centered care. Our 286-bed level II trauma center has provided care in Montana and the surrounding area since 1898.

   

Our Ortho Medical Trauma unit has 45 beds with 18 beds dedicated to total joint replacement patients and 27 beds designated for general orthopedic, orthopedic trauma spine and M/S patients. We are a busy unit with short length of stay performing over 1300 joint replacements and increasingly complex orthopedic trauma cases.15 of our RNs are NAON certified, and more are encouraged to complete the process to provide the best care to our patients. Our comprehensive care team (orthopedic surgeons, hospitalists and therapists) provides in-depth training to our staff and are great partners in care. The unit is beautifully set up with modern equipment and décor.

Your day 

   

As a Patient Navigator on our Orthopedic Medical Trauma Unit you know that every day is different, which is why you need to know how to:  

  • Adapt to quickly changing dynamic situations calmly and effectively

  • Work collaboratively as a team with physician partners and peers

  • Contact surgery patients, perform risk stratification and develop a plan of care, provide education on what to expect for elective surgeries including a pre-admission conversation involving PAT assessment, pre-operative educational class, and post-acute care planning

  • Conduct discharge assessment pre-operatively to identify issues that are barriers to discharge prior to surgery and work with the patient and care manager to remove barriers where possible to a discharge to home and create a post-acute care plan prior to the patient coming to the hospital

  • Document the post-acute plan and disseminate to the entire care team through the EMR 

Your experience 

We hire people, not resumes. But we also expect excellence, which is why we require:  

  • ASN/ADN from an accredited program

  • At least 2 years Ortho Medical Trauma RN experience

  • BSN from an accredited program required within 48 months of hire

  • Current RN license to practice nursing in the state of Montana

  • Current BLS
