Job Summary: Reports to the Director CV Informatics & VAD Program. The Coordinator-VAD Program works with the Director to case manage and coordinate all aspects of the VAD (Ventricular Assist Device) program including Destination Therapy by working with the patient and family to provide education and collaboratively establishes a plan of care, starting from the initial evaluation and following the continuum of care including pre procedure, post procedure, and continued follow-up post VAD implant. Serves as a VAD resource and a liaison among physicians, staff, patients, families, and payers to ensure effective utilization of resources, education and coordination of care. Ensures maintenance of established electronic database, and submits patient demographic, medical and progress report information within designated time frames as required by local, state, and federal agencies. Participates in the department quality improvement program and strives to optimize patient outcomes through patient education and clinical expertise. Participates and coordinates VAD specific LLU-Institutional Review Board approved research initiatives. May include on-call responsibilities on nights, weekends, and holidays to assist in assuring 24/7 access/coverage for VAD patients. Performs other duties as needed. Job Specifications: Able to read; write legibly; speak in English (and Spanish preferred) with professional quality; use computer, printer, and software programs necessary to the position, e.g., Word, Excel, Power Point, Access, CERNER, McKESSON; operate/troubleshoot basic office and patient care equipment required for the position; perform required patient care activities related to the position. Able to relate and communicate positively, effectively, and professionally with others; provide leadership; be assertive and consistent in following and enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English (and Spanish preferred) in person, in writing, and on the telephone; think critically; perform math and statistical functions necessary to the position; manage multiple assignments effectively; compose written material; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail; work independently with minimal supervision. Bachelor of Science Degree in Nursing or in process required. Master’s in Nursing preferred. Minimum three years of experience as a Staff Nurse, Emergency Medical Technician (EMT) or Cardiothoracic Surgical Nurse required (any combination of Peds and Adult accepted). Five years acute hospital experience preferred. VAD and Cardiac ICU experience preferred. Active California Registered Nursing (RN) license required. Basic Life Support (BLS) Certification required. Advanced Cardiac Life Support (ACLS) certification required. Valid California Driver’s license required. Good driving record and automobile insurance required. Provides on-call coverage according to department schedule and needs which includes nights, weekends, and holidays.
